Treatment

Medication

Your prescribed medication must be listed as a covered medication for your disease fund. You can check the list of covered medications under your specific disease fund's formulary.

Health Insurance

You must have health insurance that covers your qualifying medication or product. It is important to note that health insurance requirements may vary by disease fund.

Income

Your income must be at or below the federal poverty level specified by the disease fund. You may be asked to verify your income during the application process.

Residence

You must reside and receive treatment in the United States or US territories. US citizenship is not a requirement for receiving a PAN grant.

Applying for a PAN grant

Application Process

You can apply for a PAN grant online through the PAN portal or by phone. If you have previously applied for a PAN grant or joined a disease fund waitlist, you can apply online. Otherwise, you must apply by phone by calling 1-866-316-7263, Monday through Friday, 9 am to 5:30 pm ET.

Before applying, ensure you have the following information:

• Your diagnosis and medications
• Your health insurance carrier and insurance member ID
• Your personal and contact information, including date of birth, mailing address, email address, and social security number
• Your adjusted gross annual household income
• Your provider's contact information, including name and phone number

After Applying

Once your grant is approved, you will receive an official welcome letter and PAN identification card via email or mail within five to seven business days. If you received a transportation grant, you will also receive a prepaid Visa debit card within 15-20 business days. You can then start using your grant funds right away.

Managing a PAN grant

• You are currently receiving treatment for the disease named in the disease fund.
• Your prescribed medication is listed as a covered medication for your disease fund.
• You have health insurance that covers your qualifying medication or product.
• Your income falls at or below the federal poverty level specified by the disease fund.
• You reside and receive treatment in the United States or US territories.

Applying for a Grant

Once you have confirmed your eligibility, you can apply for a PAN grant. This can be done online through the PAN portal or by phone. If you have previously applied for a PAN grant or joined a disease fund wait list, you can apply online. If not, you must apply by phone. The number to call is 1-866-316-7263, and phone lines are open Monday through Friday, 9 a.m. to 5:30 p.m. ET.

Using Your Grant

When your PAN grant is approved, you can use your benefits immediately. You will receive an official welcome letter and PAN identification card via email or mail within five to seven business days. If you have been approved for a transportation grant, you will receive a prepaid Visa debit card in the mail within 15-20 business days.

You choose when and where to use your grant for eligible covered expenses. You can use your grant at different pharmacies or with other providers without impacting your grant status. Just provide your PAN member ID and letter to the new provider or pharmacy.

Keeping Your Grant Active

To keep your grant active, you must submit a claim or reimbursement every 120 days during your 12-month enrollment period. If you only need treatment once or twice a year and you receive an alert from PAN indicating that you must use your grant soon, you can contact PAN to discuss an exception.

Applying for More Funds

If you need more money to pay for medication before your eligibility period ends, you can apply for additional funding if the fund is open. You can apply for additional funding online through your PAN portal account or by calling 1-866-316-7263, Monday through Friday, 9:00 a.m. to 5:30 p.m. ET.

Applying for a Renewal Grant

If your initial 12-month grant eligibility period has ended or is about to end within 30 days, and you need funding for another 12 months, you may apply to renew your grant if the fund is open. You can apply for a renewal grant online through your PAN portal account or by phone.

Additional funding and renewal grants

PAN Foundation grants provide a fast and reliable way to pay for out-of-pocket medication costs, health insurance premiums, and transportation expenses. The initial PAN grant provides funding for 12 months. If you require additional funding, you can apply for it within the same initial 12-month grant eligibility period if you have exhausted your funds and your balance is $0. It is important to note that additional funding only applies to copay and health insurance premium grants.

You can apply for a renewal grant if your 12-month grant eligibility period has ended or is about to end within 30 days. Renewal grants are applicable to all types of PAN grants: copay, health insurance premium, and transportation. If approved, a renewal grant will provide funding for another 12-month eligibility period.

It is important to note that additional funding and renewal grants are not the same. Additional funding is intended to provide support within the initial grant period, while a renewal grant starts a new eligibility period.
