If you are an 'individual' applicant making a "PAN Change Request Application", you should affix two recent colour photographs (3.5 cm x 2.5 cm) in the space provided in the acknowledgement. The photograph should not be stapled or clipped to the acknowledgement. The clarity of the image on the PAN card will depend on the quality and clarity of the photograph affixed to the form.
Your signature or left-hand thumb impression should be provided across the photo affixed on the left side of the acknowledgement, in such a manner that a portion of the signature or impression is on the photo as well as on the acknowledgement receipt.
